We see all patients on an appointment basis and ask that you call and schedule an appointment in advance. We also ask that if you must cancel your appointment that you give us a 24-hour notice so that your previously reserved time can be made available for another patient.
We accept most insurance. If you have insurance benefits, please bring your plan information with you on your first visit. We will be happy to submit your insurance forms for you. All co-pays or deductibles will be due at the time services are rendered, and it will be your responsibility to pay any remaining balance after insurance benefits have been credited. We accept Visa, MasterCard, American Express, Discover and CareCredit.
